Saker Concierge Clinic provides healthcare services exclusively to adults 18 years of age and older.

This website is intended solely for individuals who are at least 18 years old. Individuals under the age of 18 should not submit personal information through this website.

A physician-patient relationship is not established merely through use of this website or submission of information through the website. The Practice will separately notify an individual if and when the Practice has accepted the individual as a patient. Completion of applicable registration materials, consent documents, and any Membership Agreement or other service agreement may be required before services are provided.

This website may permit individuals to request appointments, inquire about membership, submit enrollment requests, or initiate payment for concierge medical services.

Submission of an online inquiry, appointment request, membership application, payment, or other electronic communication does not guarantee acceptance into the Practice, establish a physician-patient relationship, or create any obligation for the Practice to provide medical services.

Enrollment in Saker Concierge Clinic becomes effective only after the Practice has:

• Reviewed and accepted the individual’s enrollment.
• Determined that the Practice is able to provide appropriate care.
• Received all required signed agreements and consent forms.
• Received any applicable membership or service fees.

The Practice reserves the right, in its sole professional discretion and consistent with applicable law, to decline enrollment or delay acceptance pending additional information.

If any payment is collected in connection with an enrollment request before the Practice accepts the individual as a patient, such payment does not constitute acceptance, and any refund will be handled in accordance with the applicable payment, enrollment, or Membership Agreement terms.

The Practice does not continuously monitor website submissions, email, or patient portal messages. If you have an urgent medical concern that is not an emergency, contact the Practice using the telephone number or other communication method provided to established patients or seek care from an appropriate urgent care facility or other healthcare provider.

Appointment requests submitted through this website are requests only.

Submission of a scheduling request does not guarantee:

• Appointment availability.
• Same-day appointments.
• Acceptance as a patient.
• Membership enrollment.
• Physician availability at a requested time.

Appointments remain subject to physician availability, clinical appropriateness, scheduling considerations, and Practice acceptance.

Our website provides access to the secure Cerbo Patient Portal for established patients. The patient portal permits eligible patients to securely:

• Exchange secure messages.
• Access portions of their medical record.
• Review laboratory results.
• Complete medical forms.
• Request appointments.
• Review billing information when available.

Users agree to:

• Maintain the confidentiality of their login credentials.
• Notify the Practice immediately of any suspected unauthorized access.
• Use the portal only for lawful purposes.
• Provide accurate information.

Protected Health Information transmitted through the Cerbo Patient Portal is governed by our Notice of Privacy Practices and applicable federal and state privacy laws. Access to and use of the patient portal may also be subject to separate terms, conditions, and privacy practices established by the applicable patient portal provider.

While reasonable efforts are made to maintain system availability, Saker Concierge Clinic does not guarantee uninterrupted access to the patient portal and shall not be liable for temporary outages, maintenance, internet interruptions, or other events beyond its reasonable control.

Electronic communications, including website submissions, email, and secure patient portal messages, are intended solely for routine, non-urgent matters. Such communications may not be reviewed immediately, and the Practice does not guarantee any particular response time.

Do not use electronic communications for emergencies or other time-sensitive medical concerns. If you are experiencing a medical emergency, call 911 or proceed to the nearest emergency department. Established patients should use the communication methods designated by the Practice for clinical matters. The availability of electronic communication does not guarantee physician availability or a response outside the availability parameters established in the applicable Membership Agreement or other patient materials.

Certain services may allow payment through Propelr, our third-party payment processor. Payment information submitted through Propelr is processed directly by Propelr using secure payment technologies.

Saker Concierge Clinic does not intentionally store complete payment card information on this website. Users are responsible for reviewing Propelr’s applicable terms and privacy practices.

The Practice operates on a self-pay basis for the services identified on this website and does not submit claims to insurance on a patient’s behalf except as expressly stated by the Practice.

Membership fees and other charges may not be covered or reimbursed by Medicare, Medicaid, commercial insurance, health savings accounts, flexible spending accounts, or other third-party payors.

Patients are responsible for understanding their own insurance coverage and for payment of all amounts owed to the Practice. Any information provided by the Practice regarding potential insurance or tax-advantaged account reimbursement is for informational purposes only and is not a guarantee of coverage or reimbursement.

The terms governing membership fees, services included in membership, cancellation, refunds, and other payment obligations are set forth in the applicable Membership Agreement.

Descriptions of concierge memberships, standalone services, wellness programs, pricing, physician availability, and other offerings on this website are provided for informational purposes only.

Actual services are governed exclusively by the applicable Membership Agreement, informed consent documents, and other agreements executed between the patient and the Practice.

In the event of any conflict or inconsistency between information appearing on this website and an executed Membership Agreement or other written agreement between a patient and the Practice, the terms of the executed agreement shall control.

The Practice reserves the right to modify pricing, membership offerings, services, and website content without prior notice unless otherwise required by law or written agreement.

Use of this website is also subject to our Website Privacy Policy. Information constituting Protected Health Information is handled in accordance with our Notice of Privacy Practices and applicable law. The Website Privacy Policy and Notice of Privacy Practices address different categories and uses of information and should be reviewed separately.
